Your first visit with Dr. Megan Welker will include:

• A thorough review of your dental and medical history.
• X-rays of your teeth and mouth
• Complete oral exam of your teeth and gums, and to check for any
• cavities or signs of tooth decay.
• You may also be able to have your teeth cleaned at your first visit.
• Lastly, Dr. Welker will provide you with her recommendations that
• may include a return visit for dental treatments.

As a first time patient, you will be asked to fill out new patient paperwork. In order to save you time, please take a moment to print and fill out the New Patient Registration paperwork below. Please make sure to bring them on your first visit to our office, as well as your dental insurance card if you have dental insurance.
